Part of Parkview Hospital’s community health improvement program, these clinics provide medical, dental, optometry, WIC, social services, and insurance enrollment— all in one central location.  Services are available to anyone regardless of their ability to pay.  These clinics offer a sliding fee discount to anyone who has not met their insurance deductible, whose insurance does not cover the service provided, and anyone uninsured.  Prenatal and pediatric care is also included among the services offered.

This practice is committed to providing a holistic approach to women’s health, working cooperatively with a woman’s fertility. Each midwife at Saint Joseph OB/GYN Specialists & Midwifery specializes in natural birth while also providing primary healthcare for women of all ages. Together with the obstetricians/gynecologists and partner physicians at Saint Joseph Health System, they provide a continuum of care for their patients.

The Family Medicine Center functions as a private office with regularly scheduled office hours. In all cases, patients are selected as family units in order to provide the experience of caring for an entire family. Their proximity to the hospital increases convenience and access for the medical residents and faculty who serve at the clinic. In addition, it gives their patients access to expanded care, including laboratory, X-ray, pharmacy and a full array of other outpatient services within the Medical Office Building and connected hospital. Their clinical pharmacist organizes a program which provides a variety of prescription medications to patients at a reduced cost.

The Sister Maura Brannick Health Center is a primary care clinic that targets medically underserved patients in St. Joseph County. The clinic’s workforce is made up mostly of volunteers, including students, nurses, primary care physicians, and specialists.

 

To be eligible for treatment, patients must be residents of St. Joseph County and completely uninsured. They cannot be covered by Medicare, Medicaid, or any private or employer-based insurance, and they must have an income that falls below 150% of the poverty level.

Our Lady of the Rosary is an extension of St. Joseph Health System, designed to serve insured community members on the west side of South Bend. It is a primary care practice offering family medicine, prenatal and obstetrical care, as well as laboratory and X-ray services.

Indiana Health Centers, Inc. (IHC) is a Federally Qualified Health Center (FQHC) that provides affordable, patient-centered health care to underserved areas of Indiana. IHC provides medical, dental, and behavioral health care to all who seek care, regardless of their ability to pay.  IHC health centers serve people who have Medicaid, Medicare, HIP, most commercial insurance companies, and provides a sliding fee scale for those without insurance.

St. Martin’s Healthcare, Inc. provides quality medical and dental care for acute and chronic conditions, optometry and medication assistance services to uninsured and underinsured residents in DeKalb and Noble counties. It is a full-service clinic that meets the needs of its residents and is impacting many individuals and families who would otherwise have no access to healthcare. All services provided at the clinic are at no charge to their patients.

Hannah’s House is the only maternity home in the Michiana region. It is a faith-based agency offering a continuum of shelter, staff support and programming for the physical, emotional and spiritual well-being of pregnant and parenting single/homeless young women. Their 24-hour staff offers structure, safety, mentoring and accountability, as well as access to existing community resources for pregnant women and single moms.

 

Their transitional, independent housing across the street from the maternity home is available for women who complete their maternity program. For six to 12 months, residents learn to adjust to the demands of being a working, single mother with the familiar support of Hannah’s House staff.

A Mother’s Hope is a refuge for homeless, pregnant women that strengthens their opportunities for stability.

 

Their residential maternity and parenting program is for pregnant homeless women who want a new start. At A Mother’s Hope, residents have the opportunity to participate in a goal-oriented program that offers a supporting and safe living environment during pregnancy and after baby is born.  

 

A Mother’s Hope assists residents with obtaining health insurance, finding a doctor, attending prenatal appointments, nutrition, safe sleep education, mental health support, addiction services referrals and other medical referrals as needed. Residents also participate in individual and group counseling on site and received weekly education on various topics such as baby care, parenting, finances, healthy relationships, and life skills. Residents are assisted, supported and guided with securing and maintaining adequate employment. All residents are required to be employed, in a job skills program, seeking employment, or in school.
